Information gathering and bedside tests focuses on information obtained through (carer or patient) interview or simple bedside tests that can assist with providing care. You will learn about the information nurses need to gather initially (such as personal history and biography) and also routinely throughout the person's hospital stay, relevant to their functional performance and ability to participate in care. Learning Outcomes

After completing this module, staff should be able to:
  • Recognise that information gathering is related to successful care of the patient with dementia and management of their behavioural symptoms.
  • Identify and use informal tools for information gathering about the patient’s personal history and current functional status.
  • Administer at least three bedside tests.
  • Identify how you would include information from bedside tests in patient progress notes.
